Insurance and Billing

All co-payments and unsatisfied deductibles must be paid at the time of service. By contractual law, your insurance company requires us to charge for, and you to pay for, all required co-payments, coinsurances, deductible and non-covered services.


We accept major insurances including the following:

  • Straight MD Medicaid

  • Medicare

  • United Healthcare (Commercial)

  • Aetna

  • Amerigroup

  • Anthem

  • Blue Cross Blue Shield

  • Cigna

  • GEHA

Please note, if you have an HMO, you are responsible for obtaining a referral from your primary care physician (PCP).  


Appointment Policy

We do our best to predict how long each patient will need with the doctor during their visit. Despite this, as cardiology is a field prone to life and death emergencies requiring us to prioritize care, occasionally there may be delays and cancellations in the schedule and we ask for your patience and understanding if and when they occur. We realize that if should you ever find yourself as the one in need of our emergency care, you would be grateful that others understood while waiting. We appreciate your understanding and will try to accommodate your needs as best as we can.

If you need to reschedule or cancel an appointment, we certainly will understand, but please kindly notify us at least 24 hours in advance to avoid a $50 no-show fee.

Remember that a no show appointment or same day cancellation takes an opportunity away from those who are need of an appointment. And maybe next time, that will be you.


Prescription Refill Policy

Good medical practice (as well as state law) dictates that patients be re-evaluated on a periodic basis prior to prescribing or refilling medications. This means that prescriptions cannot be indefinitely refilled without reevaluation. You may be asked to schedule a follow-up visit when you or your pharmacy requests a refill. If this is the case, we will make every effort within reason to assure that your medication regimen is not interrupted or that your health is not put at risk, however you must keep your follow-up appointment. Crawford-Green Cardiology refills prescriptions every Friday.

So that patients can get their medications in a timely fashion, there are some things our patients can do to work with us.

  • Watch medication refill dates – ask for your refills four to five business days before they run out (or several weeks if sending away for your prescriptions).

Plan ahead! Last-minute requests can cause problems that can result in a delay in getting your medications.

  • Call your pharmacy directly to ask for refills. We can only accept electronic or fax requests from pharmacies to ensure the accuracy of your prescriptions. Except in specific situations, we need to hear from them as we cannot routinely call them.

  • Ask pharmacies to electronically file or fax us your request.  We strongly recommend that you ask your pharmacy to electronically send or fax us requests immediately. Please confirm with your pharmacist that your prescription is ready.


Forms and Fees

There is a charge or all forms. Please allow 7-14 business days for completion of all form(s).

  • Handicap Parking - $25.00

  • Metro Access - $50.00

  • Letters (1 page) - $50.00

  • Medicaid Application Forms - $65.00

  • Social Security Disability Forms - $150.00

  • Commercial Disability Forms - $100.00 initial month, $50.00 subsequent months

  • FMLA Forms - $100.00

  • VA forms/letters - $150.00

All forms must be left with the Front Desk, even if you have a scheduled appointment.

Payment must be made with the Front Desk before forms are released.

All HMO insurance MUST have their forms completed by their Primary Care Physician.
